Delayed running of trains
I understand that the Guard and the Engine Drivers shall maintain a log book for the entire trip.
From there identify the reasons for the delay. List down the reasons, do analysis and identify the root causes (Do Why Why Analysis).
Eliminate the root causes, wherever possible across Indian Railways and ensure that trains are running on time.
Also continue to monitor, how good the actions taken and the problem repeated again or not. If repeats, then do repeated root cause analysis and take actions.
This is proven Japanese concept.

As far as I know, it is the responsibility of the guard to record the time taken by train to cover the distance between two station. He has the normal time taken by the train to cover that distance and he records the loco loss or loco gain when the train takes more time to cover that distance or vice-versa.

Most of the time ground staff and running staff know the reason/ likely reason. Problem is solution implementation. Several examples like GZB - NDLS, RE-DEC sections, there is congestion of traffic. Resources in the form of available lines are limited to 1 UP , ! DN line. SO, if there is a slower train in the section, there is cascading effect.. HWH, BCT and CSMT suburban section as at least 2 up and two down, often many more. What about resource development in DLI division? Zero. GZB_NDLS line which was called GAL loop those days was developed as additional pair of lines in late 1960s. Then-- full stop. DLI-RE section was converted from MG to BG, but no addition. Maximum problem is always at terminal or large stations where congestion increases many folds and to show trains on time, additional time is allotted, but still trains get late.
