How to close my PF account without UAN number?

Hi, Good Morning to all. This is Rajesh from Chennai. Previously (2013-2015) worked in Mumbai. Due to my personal issues i quit my job to take care of my family. Now i am trying to close my EPF account. But it is asking for a UAN number. During my working period over there, they didn't provide any UAN. Can any one please help me to close my EPF account without UAN number and Employer signature? Thanks in advance.
