First you have to submlt name correction form alongwith form 19 and 10c after getting authorised signature from your previous employer then only you can withdraw your pf accumulations. more
Please contact your past employer and get a letter signed by them giving evidence of your name , date of birth and GET IT CORRECTED FIRST and after that also apply for withdrawal in from no 13 duly signed by your past or present employer. more
Hi Gaurav. if you are working with the same company, it is good idea to go through HR and have the name correction done. Else you should visit EPF office and find the solution. Probably with voter or Aadhar card. more
