Root Cause of Indore-Patna accident

According to what I have read, the root cause of Indore-Patna derailment was insensitive TTEs and lack of pro active system.

When passengers from S1 and S2 approached TTEs about unusual thunderous sounds, the TTEs instead of calling some hotline or technical assistance ignored what passengers were saying. These 2 bogies are the ones that derailed leading to so many deaths and injuries.

Suggest the various users, railway staff that is part of this circles to share how a better process can be evolved so in future TTEs, attendants, rpf dont ignore what ppl are saying. also hot line may be constituted to report such faults. more

Some railways official are honest and might be knowing the reason for the accidents and derailing .Moreover might have the idea from their experience what to do to stop these type of derailing & accidents. There is no ways to convey the same to reach to the decision making authority. An ordinary employee can give best solution to the problem. Honorable railways minister should invite suggestions from concerned employees to give their ideas and best ideas should be recognised and rewarded. There should be a prescribed lifetime for any railway coach after which the coach should be taken out from service. Railway officials should made to feel through noble thoughts/ slogans etc departmentally that the passengers are human being and have near and dear ones like them. One who suffers only knows the pain. more  
It is possible to capture the pattern of vibrations in a wheel through a mobile phone like device and to get that transmitted in real time to Driver and Guard of the train and also to a remotely stationed Safety Engineer. more  
Better, the use of technology a vibration meter can be affixed as part of coach-undercarriage that reads to guard and engine driver continuos data feed. Defined thresh-hold and variations are defined for each coach depending on age of the coach. Such systems must be brought tro fore while allowing TTE's etc to use their skills in what their defined roles are. Defining with clarity, Operational aspects from Commercial are essential for safety. more  
We are dreaming of Bullet Trains when we cannot control the running of normal trains. Mr. Prabhu may be an honest man but Railway is full of dishonest & people never liking to work unless forced to do. Everyone puts blame on others & show them blemishless. Parliament is busy with ho hullah against re -monetisation & no one is bothered about the precious lives lost. Government thinks that their responsibility gets over once they announce the Government's help. News Papers & Media also make their appearance for few days & after that everything goes in cold storage. People also have short memories & they also forget very fast except the people who have lost their precious near & dear ones. Some must have lost their only earning member, husband might have lost wife, wife must have lost husband & so on & so forth. This is not the end of it. We cannot say that in future no such thing will happen. Budget will come, fare will increase, hardly any thought will be given on the Security & Safety of people, making the tracks crack worthy, installation of electronic warning meters which can detect the faults at a length & give you prior indication of danger ahead. Ministers are busy in filling their pockets, pleasing their constituencies people, chamchagiri of party big bosses & who cares if common man loses his life? Unless capital punishment is given to these offenders nothing will improve. Maintenance is done only when there is a big break down but there is no chart for preventive maintenance. more  
Responsibility must be fixed on TTEs, Attendnats, RPF ,etc. who are accompanying the running trains in addition to a special desk to be set up in the Guard Compartment to hear the passengers complaints / suggestions and to take corrective measures. In the instant case if the TTEs would take care the passengers' complaints/views , the derailment can be avoided. more
